Job Description:
A company is looking for a Care Team Associate to support Care
Management processes and assist clinical staff remotely.
Key Responsibilities
Document patient interactions and manage Care Coordination
inquiries across various communication channels
Coordinate Patient Care Committee meetings and manage daily census,
including scheduling and follow-up appointments
Provide administrative support to clinical staff and maintain
knowledge of health plan benefits and regulations
Required Qualifications
High School Diploma/GED
Must be 18 years of age or older
2+ years of administrative support experience
Knowledge of medical terminology
Advanced knowledge of Microsoft Office products
